MUST REPLACE MISSING COVING AT WALL OUTSIDE FRONT EMPLOYEE RESTROOM, UNDER WAFFLE MAKING STATION AND UNDER HOOD AT RIGHT SIDE OF DISH MACHINE. MUST SEAL GAP IN FLOOR AROUND BASE OF MOP SINK AND AT WALL UNDER DISH MACHINE. OBSERVED EXTENSION CORDS RUNNING ACROSS FLOOR IN FRONT PREP LINE. MUST REMOVE CORDS SO THE FLOOR IS BE EASILY CLEANABLE. OPERATOR REMOVED EXTENSION CORDS DURING INSPECTION.
MUST REPAIR WALLS TO BE SMOOTH, EASILY CLEANABLE AND FREE OF UNNECESSARY GAPS AND CRACKS IN THE FOLLOWING AREAS: UNDER THE HOOD IN REAR PREP, BEHIND/BELOW THE DISH MACHINE AND UNDER THE MAIN 3 COMPARTMENT SINK. MUST REPAIR THE HOLE IN THE WALL NEAR THE COMPRESSORS IN THE BASEMENT FOYER. MUST REPAIR FRAME OF KITCHEN/DINING ROOM WINDOW WHERE TILES ARE MISSING AND UNFINISHED DRYWALL IS EXPOSED.
MUST REPLACE CRACKED LIGHT SHIELDS IN REAR PREP AREA.
MUST PROVIDE BACKFLOW PREVENTION DEVICE(S) FOR COFFEE/HOT WATER/HOT CHOCOLATE MACHINES AT FRONT COUNTER. MUST INSTALL EXHAUST FAN IN EMPLOYEE RESTROOM (NEAR FRONT COUNTER) THAT IS EXHAUSTED TO THE OUTDOORS.
OBSERVED LARGE AMOUNT OF CLUTTER IN SIDE BASEMENT STORAGE ROOM INCLUDING SPARE RESTAURANT EQUIPMENT, FURNITURE, BOXES AND MANY OTHER ITEM. THE STORAGE ROOM IS PACKED WITH THESE ITEMS FROM WALL TO WALL, SUCH THAT A MAJORITY OF THE ROOM (INCLUDING ALL 4 CORNERS) IS INACCESSIBLE FOR CLEANING AND PEST CONTROL MONITORING AND IS CREATING A POSSIBLE PEST HARBORAGE AREA. ALSO OBSERVED CLUTTER (EXCESS RESTAURANT EQUIPMENT, MAINTENANCE MATERIALS) IN BASEMENT CLOSET NEAR REAR STAIRS. INSTRUCTED TO REMOVE UNNECESSARY ARTICLES AND STORE REMAINING ITEMS AT LEAST 6 INCHES OFF THE DOOR AND PROVIDE ACCESS TO ALL CORNERS, WALLS AND AREAS OF BASEMENT STORAGE.
PROOF OF COMPLETED FOOD HANDLER TRAINING NOT AVAILABLE AT TIME OF INSPECTION. INSTRUCTED THAT ALL FOOD HANDLERS MUST MEET MINIMUM FOOD HANDLER REQUIREMENTS AND MAINTAIN DOCUMENTATION OF COMPLETION ON SITE. ALL NEW FOOD HANDLERS MUST BE TRAINED WITHIN 30 DAYS OF EMPLOYMENT.
