6-202.13: OBSERVED A LARGE RODENT STICKY TRAP ON THE SHELF UNDER THE FOOD PREP TABLE NEXT TO THE MICROWAVE IN THE REAR FOOD PREP AREA. INSTRUCTED TO REMOVE STICKY TRAP, ONLY USE PRODUCTS FROM PEST CONTROL COMPANY, AND REFRAIN FROM STORING ON MENTIONED SURFACES.
4-201.11: OBSERVED THE HAND SOAP DISPENSER IN POOR CONDITION IN THE REAR FOOD PREP/ DISHWASHING AREA. ALSO, FOUND A BOWL OF SOAP WITH A SPOON BEING USED AS A SOAP DISPENSER. INSTRUCTED TO PROVIDE AN ADEQUATE SOAP DISPENSER AND MAINTAIN.
4-601.11(C): OBSERVED THE LOWER SHELVES UNDER THE FOOD PREP TABLES IN NEED OF CLEANING. INSTRUCTED TO REMOVE GREASE BUILD UP AND MAINTAIN.
