NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS

Found/observed front entrace/exit door not rodent proofed, door had 1/4 inch gap on bottom of door(west), also above had no proof of pest comtrol service, log book, at this time of license inspection.
