FOUND NO CITY OF CHICAGO CERTIFICATE OR CERTIFIED FOOD MANAGER ON SITE WHILE SERVING/HANDLING POTENTIALLY HAZARDOUS FOODS (I.E. HAM, SAUSAGE, EGGS, MILK, ETC.) INSTRUCTED TO HAVE BOTH ON THE PREMISES AT ALL TIMES. SERIOUS VIOLATION 7-38-012
MUST FIX AND MAINTAIN THE CRACKED/DAMAGED WALL BY THE WASHBOWL IN THE 1ST TOILET ROOM IN THE BASEMENT.
MUST ELIMINATE CLUTTER AND ORGANIZE AREA IN THE 1ST FL TOILET ROOM AND IN THE BASEMENT BY THE TOILET ROOM.
The walls and ceilings shall be in good repair and easily cleaned. MUST CLEAN ALL UNCLEAN WALLS AT PREP AREA/DISHWASH AREA AND MAINTAIN.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. OBSERVED UNNECESSARY CLUTTER UNDER STAIRCASE. MUST REMOVE/RE-ORGANIZE ALL ITEMS UNDER STAIR CASE.
All food not stored in the original container shall be stored in properly labeled containers. Found bulk food not labeled.(large container of sugar)
All employees shall be required to use effective hair restraints to confine hair. Foodhandler not wearing hair restraint.
