MUST CLEAN INTERIOR PANEL OF THE ICE MACHINE TO REMOVE SLIGHT BUILD-UP.
MUST REPAIR LEAK AT THE MIDDLE PORTION OF THE SPRAYER AT THE 3-COMP SINK. AT THE RESTROOM HAND SINK MUST ADJUST TEMP VALVE SO AS TO HAVE HOT WATER RUN FOR A MINIMUM OF 15 SECS AT ALL TIMES AT 85F MINIMUM.
MUST BETTER ORGANIZE THE AREA TO THE RIGHT HAND SIDE OF THE HAND SINK LOCATED ON THE NORTH WALL. ALSO THE AREA IN FRONT OF THE WALK-IN COOLER MUST REMOVE EXCESS CARD BOARD BOXES FROM ABOVE AREA.
CLEAN/SANITIZE COUNTER TOPS AT FRONT BEHIND SERVICE COUNTER OF SPILLS AND FOOD DEBRIS.
CLEAN FLOORS IN FRONT PREP AREA ALONG WALLBASE AND BEHIND HEAVY EQUIPMENT, UNDER STORAGE SHELVES AT REAR AND IN WALK-IN COOLER/FREEZER.
REMOVE CLUTTER UNNECESSARY ARTICLES IN FRONT STORAGE AREA. ORGANIZE AND MAINTAIN AT ALL TIMES.
NOTED 20 LIVE FLIES ON THE WALL NEXT TO THE REAR EXIT DOOR ALSO IN THE SAME AREA SOME LIVE FLIES INSIDE A GARBAGE CAN. NOTED 6 LIVE FLIES IN THE REAR STORAGE AREA. ALSO NOTED 10 ON THE WALL AND GLASS IN THE HALLWAY TO THE TOILET ROOM. INSTRUCTED MANAGER TO ELIMINATE FLY PROBLEM AND CLEAN AND SANITIZE AFFECTED AREAS. SERIOUS CITATION ISSUED #7-38-020.
NO CERTIFIED FOOD MANAGER ON DUTY WHILE POTENTIALLY HAZARDOUS FOODS ARE BEING PREPARED AND SERVED SUCH AS HAM, EGG, CHICKEN SALAD SANDWHICHES AND COFFEE DRINKS MADE WITH MILK. A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. CITATION ISSUED #7-38-012.

All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. NO THERMOMETER INSIDE STAND UP MILK REACH IN COOLER. MUST PROVIDE THERMOMETER FOR COOLER.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. DRY STORAGE ROOM CLUTTERED. MUST ELEVATE ALL ITEMS 6" OFF FLOOR OR REMOVE ITEMS TO PREVENT POTENTIAL PEST HARBORAGE.

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Must fix leaking water supply lines behind the ice machine & at coffee station. Observed water leaking profusely. The pan used to catch water is overflowing. Unit tagged, must fax letter to (312)746-4240 to un-tag unit.
