OBSERVED VENTILATION HOOD ABOVE MAIN COOK LINE TO BE HEAVILY-SOILED WITH GREASE AND OTHER DEBRIS. INSTRUCTED TO DEEP CLEAN SURFACES OF VENTILATION HOOD SYSTEM TO ELIMINATE BUILD UP OF GREASE AND DEBRIS AND MAINTAIN NONFOOD-CONTACT SURFACES OF EQUIPMENT CLEAN TO SIGHT AND TOUCH.
OBSERVED CLUTTER AND UNNECESSARY ITEMS AND EQUIPMENT THROUGHOUT RESTROOM AREA FLOOR. INSTRUCTED TO RELOCATE CLUTTER AND REMOVE ITEMS UNNECESSARY TO FOOD OPERATIONS FROM THE PREMISES AND MAINTAIN ALL AREAS FREE FROM CLUTTER OR UNNECESSARY ITEMS.
OBSERVED PERSON-IN-CHARGE WITH CITY OF CHICAGO CERTIFIED FOOD MANAGER TRAINING MISSING ADDITIONAL ALLERGEN AWARENESS TRAINING AND CERTIFICATE ON-SITE AT TIME OF INSPECTION. INSTRUCTED TO PROVIDE ALL CERTIFIED FOOD MANAGERS WITH ADDITIONAL ALLERGEN AWARENESS TRAINING AND MAINTAIN CERTIFICATE(S) ON-SITE AT ALL TIMES.
