All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED LIVE FRUIT FLIES ON THE WALLS, CEILING AND FOOD EQUIPMENT IN THE REAR PREP AREA, WASHROOMS AND DINING AREA. APPROXIMATELY 25 LIVE FRUIT FLIES WERE OBSERVED. ALSO OBSERVED FLY STRIPS ON THE WALLS OF THE PREP AREAS. MANAGEMENT INSTRUCTED TO REMOVE. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. OBSERVED THE 3 COMPARTMENT SINK IMPROPERLY MAINTAINED. THE RIGHT COMPARTMENT OF THE SINK HAD AN OPENING ALONG THE BOTTOM SEAM OF THE COMPARTMENT SO THAT IT CANNOT BE FILLED UP WITH WATER WITHOUT LEAKING. MANAGEMENT INSTRUCTED TO REPAIR OR REPLACE THE 3 COMPARTMENT SINK. SERIOUS VIOLATION 7-38-030.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. DETAIL CLEAN THE INTERIOR OF THE PREP COOLER.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. REPAIR THE BROKEN FLOOR TILE AROUND THE FLOOR DRAIN AND AT THE REAR DOOR. DETAIL CLEAN THE FLOOR OF THE REAR DRY STORAGE AREA.
The walls and ceilings shall be in good repair and easily cleaned. REPLACE THE BASEBOARD IN THE PREP AREA THAT IS COMING LOOSE FROM THE WALL, ESPECIALLY BEHIND THE 3 COMPARTMENT SINK AND AROUND THE WALK-IN COOLER. DETAIL CLEAN THE WALL BEHIND THE COOKING EQUIPMENT.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. MUST PROVIDE A LIGHT SHIELD WHERE MISSING IN THE REAR PREP AREA.
NO COLD WATER IS AVAILABLE AT THE REAR EXPOSED HANDSINK. REPAIR THE COLD WATER HANDLE AT THE REAR EXPOSED HANDSINK.
All employees shall be required to use effective hair restraints to confine hair. HAIR RESTRAINTS NEEDED FOR ALL EMPLOYEES IN THE FOOD PREP AREA.
