INSPECTION HISTORY
All food establishments that display, prepare, or store potentially hazardous food shall have adequate refrigerated food storage facilities. COOLER AND FREEZERS ARE OFF AT THIS TIME, MUST KEEP COLD UNITS AT 40F OF BELOW.
All food establishments shall be provided with an adequate supply of hot and cold water under pressure properly connected to the city water supply. NO HOT WATER, MUST PROVIDE HOT WATER UNDER CITY PRESSURE.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. FRONT DOOR NOT RODENT PROOFED, MUST SEAL BOTTOM AND SIDE OF FRONT DOOR TO PREVENT PEST ENTRY, AND NO PEST CONTROL LOG BOOK, MUST PROVIDE.
The area outside of the establishment used for the storage of garbage shall be clean at all times and shall not constitute a nuisance. MUST REMOVE DEBRIS AND CLEAN OUTSIDE AREA TO PREVENT PEST HARBORAGE.
All garbage and rubbish containing food wastes shall, prior to disposal, be stored in metal containers with tight fitting lids and shall be kept covered except when opened for the disposal or removal of garbage. MUST PROVIDE INSIDE CONTAINERS.
A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or serve. NO CERTIFIED FOOD MANAGER OR DOCUMENTS, MUST PROVIDE CITY CERTIFICATE OR ENROLLMENT LETTER UPON REINSPECTION.
All utensils shall be thoroughly cleaned and sanitized after each usage. MUST CLEAN-SANITIZE ALL EQUIPMENT, TABLES, 3PART SINK.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST CLEAN FLOORS THRU-OUT PREMISES, INCLUDING BASEMENT.
The walls and ceilings shall be in good repair and easily cleaned. MUST SEAL OPENINGS AROUND PIPES TO PREVENT PEST ENTRY.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. MUST PROVIDE LIGHT SHILELDS FOR LIGHT FIXTURES ABOVE PREP AREA AND ABOVE ICE CREAM CHEST FREEZERS.
Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. MUST PROVIDE A SELF-CLOSING DEVICE FOR WASHROOM DOOR.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST REPAIR 3PART SINK FAUCET, AND INSTALL A BACK FLOW DEVICE FOR MOP SINK.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. MUST PROVIDE THERMOMETER FOR REACH IN COOLER.
