OBSERVED APPROXIMATELY 80 DEAD GNATS ON FLOOR ALONG WALLS IN THE BASEMENT AREA. INSTRUCTED TO REMOVE DEAD OR TRAPPED INSECTS AND CLEAN AFFECTED AREA(S) AT ADEQUATE FREQUENCY TO PREVENT BUILD UP OF DEAD INSECTS AND BECOMING AN ATTRACTANT TO ADDITIONAL PESTS AND MAINTAIN.
OBSERVED WASHROOM DOOR OPENING INTO MAIN KITCHEN AREA TO NOT BE SELF-LATCHING WHEN CLOSED. INSTRUCTED TO MAINTAIN WASHROOM DOORS WHICH OPEN TO FOOD PREPARATION AREAS TO BE SELF-CLOSING AND SELF-LATCHING AND MAINTAIN CLOSED DURING FOOD OPERATIONS.
OBSERVED VENT COVER ABOVE UTILITY SINK IN THE REAR AREA TO BE HEAVILY-SOILED WITH DUST DEBRIS. INSTRUCTED TO CLEAN VENTILATION COVERS AND OTHER PHYSICAL FACILITIES AT ADEQUATE FREQUENCY TO PREVENT BUILD UP OF DUST AND OTHER SOILS AND MAINTAIN.
OBSERVED NO MECHANICAL VENTILATION PROVIDED IN THE EMPLOYEE WASHROOM. INSTRUCTED TO MAINTAIN FUNCTIONAL MECHANICAL VENTILATION OF ADEQUATE CAPACITY TO REMOVE FUMES AND ODORS FROM REQUIRED AREAS SUCH AS WASHROOMS AND MAINTAIN.
OBSERVED CITY OF CHICAGO CERTIFIED FOOD MANAGERS ON-SITE WITHOUT ALLERGEN AWARENESS TRAINING OR CERTIFICATES AT TIME OF LICENSE INSPECTION. INSTRUCTED TO PROVIDE ALL CITY OF CHICAGO CERTIFIED FOOD MANAGERS WITH ADDITIONAL ALLERGEN AWARENESS TRAINING AND MAINTAIN CERTIFICATE(S) ON-SITE AT ALL TIMES FOR INSPECTOR REVIEW.
