OBSERVED GREEN, BLACK, AND BROWN MOLD-LIKE DISCOLORATIONS IN THE UPPER COMPARTMENT OF THE ICE MACHINE DRIPPING DOWN ONTO THE ICE. BEFORE OPENING THE SECOND COMPARTMENT WITH THE SCREWDRIVER, OBSERVED A ROACH CRAWL INTO THE UPPER COMPARTMENT OF THE ICE MACHINE. MANAGEMENT INSTRUCTED TO REPLACE THE MACHINE OR REMOVE ALL DISCOLORATIONS, REPLACE ALL SOILED WATER HOSES INSIDE OF THE MACHINE, AND PREVENT ANY PESTS FROM ENTERING THE ICE MACHINE. MANAGER WAS ABLE TO DUMP THE EXISTING ICE IN THE MACHINE, DETAIL CLEAN AND SANITIZE THE INTERIOR SURFACES OF THE MACHINE BEFORE THE END OF THE INSPECTION. SERIOUS VIOLATION 7-38-005A

OBSERVED LIVE FRUIT FLIES IN THE FOOD PREP AREA, BAR, AND ALSO NEAR A WALK-IN COOLER THAT IS NO LONGER IN USE. APPROXIMATELY 85 LIVE FRUIT FLIES WERE OBSERVED ON PREMISES.
