OBSERVED GROCERY BAGS BEING USED TO STORE FOOD IN THE 1-DOOR COOLER NEXT TO THE ICE MACHINE, AND IN THE WALK IN COOLER IN THE BASEMENT. RUST ON RACKS IN THE ABOVE MENTIONED 1-DOOR COOLER. REPLACE CUTTING BOARDS IN THE REAR PREP AREA, DEEP BLACK GROOVES ARE PRESENT. ALSO, DEFROST ALL DEEP FREEZERS IN THE BASEMENT, EXCESSIVE ICE ACCUMULATION. INSTRUCTED TO DISCONTINUE USE OF GARBAGE BAGS, THEY ARE PROHIBITED. REPAIR RUST ON RACKS, SO THEY ARE SMOOTH AND EASILY CLEANABLE. REPLACE/REPAIR CUTTING BOARDS AND DEFROST FREEZERS AND MAINTAIN.

BEVERAGE STATION OUTSIDE OF THE KITCHEN. STICKY CONTACT SURFACES THROUGHOUT THE KITCHEN. INTERIOR SURFACES OF ALL DEEP FREEZERS IN THE BASEMENT. INSTRUCTED TO DETAIL CLEAN ALL AND MAINTAIN.

ALSO, FLOOR IN THE MOP ROOM IS NOT CLEAN, DIRT AND DEBRIS IN CORNERS. INSTRUCTED TO DETAIL CLEAN ALL AREAS AND MAINTAIN.
OBSERVED STAINED CEILING TILES THROUGHOUT DINING, REAR PREP, AND BASEMENT AREAS. WALL DAMAGE NEXT TO THE ICE MACHINE. HOOD AND FILTERS IN KITCHEN HAVE EXCESSIVE DUST AND GREASE ACCUMULATION. CEILING VENTS IN THE KITCHEN, AND MENS AND WOMENS TOILET ROOMS HAVE EXCESSIVE DUST ACCUMULATION. INSTRUCTED TO REPAIR ALL AND MAINTAIN.

OBSERVED LEAKY FAUCET AT 3-COMP SINK IN KITCHEN. ALSO, LOOSE FAUCET AT THE EXPOSED HAND SINK IN THE MENS TOILET ROOM. INSTRUCTED TO REPAIR AND MAINTAIN.
OBSERVED EXCESSIVE CLUTTER THROUGHOUT THE UPSTAIRS STORAGE AREA. INSTRUCTED TO ORGANIZE, AND ELEVATE ITEMS 6" OFF FLOORS TO PREVENT RODENT HARBORAGE AND MAINTAIN.
