OBSERVED NO CLEAN-UP OF VOMITING AND DIARRHEA PROCEDURE AND CLEAN-UP KIT ON PREMISES DURING INSPECTION. INSTRUCTED TO HAVE SAID PROCEDURE ALONG WITH REQUIRED EQUIPMENT FOR CLEAN-UP. PRIORITY FOUNDATION VIOLATION. NO CITATION ISSUED. 7-38-005

OBSERVED THAT SINGLE SERVICE ITEMS HAVE BEEN STORED ON FLOOR UNDER STAIRS AT LOWER DRY STORAGE ROOM. MUST STORE ALL SINGLE SERVICE ITEMS 6 INCH OF THE FLOOR ON DRY AND SAFE LOCATION

NO ONLINE STATE OF ILLINOIS FOOD TRAINING CERTIFICATES FOR EMPLOYEES.MUST PROVIDE ALL CERTIFICATES AT NEXT ROUTINE INSPECTION.
MUST DETAIL CLEAN AND SANITIZE FLOOR DRAINS AT THE COFFEE SERVICE STATION TO THE LEFT OF THE CASHIER'S. ALSO THE FLOOR AT THE CORNERS AT SAME LOCATION AS STATED ABOVE.
MUST REPAIR NON WORKING EXHAUST VENT IN THE RESTROOM.
CLEAN PREP COUNTERS AND EQUIPMENT BEHIND FRONT SERVICE COUNTER OF FOOD DEBRIS AND SPILLS.
CLEAN FLOOR BEHIND FRONT SERVICE COUNTER.
MUST CLEAN THE COUNTER TOP UNDER AND AROUND THE COFFEE MACHINE TO REMOVE SPILLS. ALSO INSIDE THE REACH IN COOLER IN THE REAR STORAGE AREA.
MUST CLEAN THE FLOORS IN THE FRONT PREP AREA INSIDE THE CADINET WITH CUT OUT BOTTOMS. ALSO ALONG THE WALL BASE AND IN CORNERS TO REMOVE DEBRIS.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Observed a leak at pipe under 3-comp sink (right side)-must fix.
A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority. Previous minors from 5/13/09/visit, not corrected at this time. ((#32-must provide a sneezeguard around the front, central prep station/counter (between 2-coffee machines). All the prepping of drinks are done at this area, except for the blenders (done on counters by the walls); also must seal around handsinks & countertops-due to water seepage.))
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. Cabinet ledges under front counters with coffee stains/beans, need cleaning; dusty fan guards of small front coolers need cleaning also.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Floors under front handsinks & behind booster heater & grease trap in rear need detailed cleaning.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. Cracked light shield in rear & flickering light in mop room must be replaced.
