OBSERVED MOP SINK FAUCET WITH ATTACHED HOSE ADAPTER. INSTRUCTED TO REMOVE AND INSTALL BACKFLOW PREVENTER DEVICE.
INSTRUCTED TO REPAIR SLOW DRAINING HAND SINK IN FRONT PREP AREA.
OBSERVED LEAVES AND DEBRIS ON GROUND UNDER AND AROUND GARBAGE DUMPSTER IN REAR OUTSIDE AREA. INSTRUCTED TO CLEAN AND MAINTAIN.
INSTRUCTED TO DETAIL CLEAN AND MAINTAIN FLOORS ALONG THE WALLS AND IN ALL CORNERS IN REAR DINING/DRY STORAGE AREA ESPECIALLY UNDER AND AROUND REACH-IN COOLERS AND FREEZER AND FLOORS INSIDE REAR BOILER ROOM.
INSTRUCTED TO CLEAN AND REORGANIZE BEHIND FRONT COUNTER SHELVES, REAR DRY STORAGE ROOM AND IN REAR BOILER ROOM. USABLE ITEMS MUST BE STORED 6" OFF THE FLOOR FOR EASY ACCESS ON CLEANING AND TO PREVENT PEST HARBORAGE.
FOOD HANDLER WITH NO PROOF OF FOOD HANDLER TRAINING CERTIFICATE. INSTRUCTED MANAGEMENT, ALL FOOD HANDLERS THAT DO NOT HAVE CITY OF CHICAGO FOODSERVICE SANITATION CERTIFICATE SHALL COMPLETE AN APPROVED FOOD HANDLER TRAINING PROGRAM.
