NO BACKFLOW PREVENTION DEVICE WAS PROVIDED AT THE ICE MACHINE WATER LINE IN THE PREP/KITCHEN AREA. MUST BE INSTALLED SO BACKFLOW PREVENTION DEVICE MAY BE LOCATED TO BE SERVICED AND MAINTAINED OR TO KNOW THE LOCATION.
LID IS REQUIRED AT GARBAGE RECEPTACLE INSIDE THE STAFF WASHROOM
GREASE ON WALLS IN PREP AREA CLEAN AND MAINTAIN.ALSO, ON FLOOR UNDER THE COOKING EQUIPMENT,AND THROUGHOUT
MISSING CEILING TILE ABOVE THE EXTERIOR OF THE WALK-IN COOLER IN PREP AREA,REPLACE. MISSING WALL TILES IN PREP AREA, REPLACED WITH DUCT TAPE. REMOVE SAID TAPE. WALLS MUST BE SMOOTH,CLEANABLE AND NON-ABSORBENT MATERIAL .
