Found food items not properly dated/labeled in coolers, reach in freezer, must date and label food items in reach in coolers, reach in freezer.

Non food contact surfaces of front storage shelving, front counter shelving not clean need cleaning, reach in cooler racks need cleaning.

Floors in kitchen prep area, front dining areas not clean need cleaning(detailed), floors in storage area rear kitchen under shelving/racks need detailed cleaning(corners).

Wall above 3 compartment sink had a large hole, peeling paint, shall be repaired/sealed, wall frame in front prep area had exposed raw wood, shall be repaired/sealed, walls in rear prep area not clean need cleaning.

Light shields that had missing end caps, shall be provided, light shields in prep areas need cleaning(detailed).

Exhaust vents(ventilation) in coolers, not clean need detailed cleaning, exhaust canopy needs cleaning.

Must provide thermoters to all cooler in order to maintain proper temperatures of 40f or less.

Reach in cooler shelving/racks that had rust, peeling paint, shall be repaired/replaced.

Non food contact surfaces of cooler shelving, cooking equipment, and storage shelving not clean need detailed cleaning(premises).

Floors under heavy equipment, cooking equipment not clean, need detailed cleaning(corners).

Walls that had peeling paint, exposed plaster in kitchen prep area shall be repaired/sealed.

Exhaust canopy had excess grease build up, needs detailed cleaning, exhaust vent(ventilation)in bathroom needs cleaning.
