OBSERVED MISSING AND DAMAGED FLOOR TILES AT BAR. INSTRUCTED TO REPAIR TO CREATE A SMOOTH AND EASILY CLEANABLE SURFACE. MUST MAINTAIN AT ALL TIMES.
OBSERVEDHOLES IN BASEMENT CEILING AND WATER DAMAGED DRYWALL CEILING ABOVE THE FOOD PREP AREA. INSTRUCTED FOOD MANAGER TO REPAIR WATER DAMAGED CEILING SO THAT IT IS SEALED, SMOOTH AND EASILY CLEANABLE IN ALL PREP AREAS.
