OBSERVED THE BOTTOM SHELF IN THE SERVICE AREA LOCATED IN THE NORTH EAST CORNER OF THE DINING AREA NEXT TO EXIT DOOR TO BE HEAVILY WATER DAMAGED AND NOT SMOOTH AND EASILY CLEANABLE. INSTRUCTED MANAGER TO REPLACE/REPAIR AND PROVIDE A NON ABSORBENT SUFACE.
OBSERVED BACKFLOW PREVENTION OF ICE MACHINE IN THE BASEMENT TO BE LEAKING ONTO THE FLOOR BEHIND THE MACHINE. INSTRUCTED MANAGER TO REPAIR/REPLACE AND MAINTAIN IN GOOD REPAIR.
OBSERVED THE EMPLOYEE BATHROOM IN THE KITCHEN TO NOT HAVE A COVERED TRASH CAN AT TIME OF INSPECTION. INSTRUCTED MANAGER TO PROVIDE A COVERED TRASH CAN IN EVERY FEMALE OR UNISEX BATHROOMS.
OBSERVED THE FLOOR BELOW THE SODA DISPNER SHELF IN THE BASEMENT TO BE SOILED WITH SPILLAGE. INSTRUCTED MANAGER TO DETAIL CLEAN AND MAINTAIN FREE OF EXCESSIVE SPILLAGE BUILD UP.

OBSERVED THE MANAGER TO NOT HAVE ALLERGEN TRAINING CERTIFICATE AT TIME OF INSPECTION. INSTRUCTED MANAGER TO COMPLETE AND MAINTAIN ALLERGEN TRAINING FOR ALL CERTIFIED FOOD MANAGERS.
