Floors in kitchen prep areas, rear storage areas not clean, need detailed cleaning(corners), missing/damaged floor tiles in dining area shall be repaired/replaced.

Missing ceiling tiles in front kitchen area, dining areas shall be provided, walls that had exposed raw wood/exposed plaster shall be repaired/sealed. Missing cove moulding in kitchen areas, dining areas shall be provided.

Exhaust vents(ventilation) in walk in cooler not clean, need detailed cleaning, exhaust vents(ventilation) in bathrooms in state of disrepair not working, must replaced/provide exhaust fans to bathrooms. Missing exhaust vent guards in dining areas shall be provided.

Foods inside walk-in cooler not labeled or dated. Instructed to label and date all foods not in original container.

Multi use utensils not properly stored. Instructed to invert utensils to prevent contamination before use.

Dry goods storage shelves not maintained. Instructed to detail clean and repaint shelves, including shelves in adjacent storage room.

Storage shelves in coolers not cleaned. Instructed to detail clean and sanitize.

Floor through-out premises not cleaned. Instructed to detail clean floor in oven unit and behind and under cooking equipment. storage room and under three compartment sink.

Ceiling, ventilation covers and light covers not cleaned or maintained. Instructed to detail clean clean ventilation covers and ceiling of dust build-up and detail clean or replace light covers.

Thermometer not provided inside pizza prep cooler. Instructed to provide thermometer visible inside all coolers.
