6-501.11: OBSERVED HOLES IN THE LOWER WALLS IN THE BASEMENT FOOD PREP AREA. HOLES WERE IN THE PIZZA PREP AREA UNDER HANDWASHING SINK AND IN DISHWAHSING AREA NEAR THE MOP SINK CLOSET. INSTRUCTED TO SEAL HOLES TO ELIMINATE POTENTIAL HARBORAGE POINTS FOR PEST AND MAINTAIN.
2-102.13: OBSERVED NO ALLERGEN TRAINING CERTIFICATE FROM CERTIFIED FOOD MANAGER ON SITE. INSTRUCTED ALL CERTIFIED FOOD MANAGERS MUST HAVE ALLERGEN TRAINING CERTIFICATE AND MAINTAIN RECORDS ON SITE.
