Found some food items not properly dated in reach in freezer, must properly date food items in reach freezer.

Non food contact surfaces of reach in cooler shelving not clean, need detailed cleaning(crevices), rear storage shelving not clean, need detailed cleaning(crevices), rach in cooler fan guards not clean, need detailed.

Floor under heavy equipment, cooking equipment , not clean, need detailed cleaning(corners).

MUST DATE AND LABEL ALL COOKED AND PREPARED FOODS INSIDE COOLERS,

ALL SINGLE SERVE ARTICLES MUST BE STORES WITH HANDLES FACING UP,

MUST PROVIDE SPLASH GUARD AT EXPOSED HAND SINK FRONT PREP AREA, BY COUNTER TABLE, AND EXPOSED HAND SINK,

MUST REPAIR FAUCET TO THREE COMPARTMENT, THATS IN POOR REPAIR, MAKING LOUD SOUNDS,

Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use.All clean single service utensils must be stored with handles up.

All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.A SPLASHGUARD IS NEEDED AT EXPOSED HANDSINK FRONT PREP AREA,RAW WOODEN SURFACES MUST BE SEALED USING A NON-TOXIC FINISH.

All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily.THE FOLLOWING REQUIRES A DETAIL CLEANING FAN COVERS INSIDE 2 DOOR FOOD PREP COOLER,INTERIOR OF DIP WELL,SHELVING BEHIND FRONT SERVICE COUNTERS.

Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated.TOILET ROOM DOORS MUST BE SELF CLOSING.

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.ELECTRIC BUFFET RANGE IN KITCHEN MUST BE UNDER VENTILATION, WHEN THE EQUIPMENT IS MORE THAN 7.5 KILOWATTS.
