INSPECTION HISTORY
All food establishments that display, prepare, or store potentially hazardous food shall have adequate refrigerated food storage facilities. MUST HAVE COOLERS AT 40F OR BELOW.
In food establishments, there shall be adequate sewage and waste water disposal facilities that comply with all the requirements of the plumbing section of the Municipal Code of Chicago. MUST PROPERLY INSTALL A MOP SINK WITH HOT/COLD RUNNING WATER AND A BACK FLOW DEVICE, AND PROVIDE A GREASE TRAP FOR 3PART SINK.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. MUST CONTRACT A LICENSED PEST CONTROL CO. AND PROVIDE LOG BOOK.
A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. MUST ENROLL FOR SANITATION COURSE AND PROVIDE LETTER, OR HAVE A CERTIFIED FOOD MANAGER WITH CITY CERTIFICATE.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST SEAL RAW WOOD SHELVES, AND SIDE PANELS UNDER FRONT SERVING COUNTER.
The walls and ceilings shall be in good repair and easily cleaned. MUST PROVIDE BASEBOARDS FOR BOTTOM OF WALLS THRU-OUT KITCHEN.
Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. MUST PROVIDE A SELF CLOSING DEVICE FOR WASHROOM DOOR.
