METAL PLATE AT WALK-IN COOLER DOOR (3RD FLOOR) IS PULLING AWAY FROM THE FLOOR, MUST BE FIXED. ALL STOCK/SUPPLIES MUST BE ELEVATED OFF THE FLOORS IN ALL DRY/COLD STORAGE ROOMS. The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.
ANY MOLDY/RIPPED CAULKING ALONG SPLASHGUARDS OF SINKS THRU-OUT PREMISES MUST BE REPLACED. OPENING UNDER HANDSINK IN CAFETERIA (LOWER LEVEL) MUST BE SEALED. WALLS AROUND GARBAGE BINS MUST BE KEPT CLEAN ALSO. The walls and ceilings shall be in good repair and easily cleaned.
FOLLOWING PLUMBING ISSUES MUST BE ADDRESSED:LEAKY FAUCET AT 3-COMP SINK AT 2ND FLOOR; SLOW DRAIN AT HANDSINK ON SAME FLOOR-MUST FIX. Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.
ANY MILK/BREAD CRATES USED FOR DRY/COLD STORAGE RACKS MUST BE REMOVED & REPLACED WITH REGULAR SHELVINGS FOR EASY ACCESS TO CLEANING/PEST MONITORING. All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors.
