OBSERVED LIQUOR BOXES STORED ON THE FLOOR IN THE WALK-IN COOLER. INSTRUCTED MANAGER TO PROPERLY STORE AND PROTECT FOOD ITEMS FROM CONTAMINATION WHILE STORED BY ELEVATING AT LEAST SIX INCHES FROM THE FLOOR ON RAISED SHELVING UNITS.

OBSERVED BROKEN HANDLE AT FAUCET IN EMPLOYEE RESTROOM. INSTRUCTED TO REPAIR AND MAINTAIN.

OBSERVED NO COVERED RECEPTACLE FOR PROPER DISPOSAL OF SANITARY NAPKINS IN EMPLOYEE WASHROOM. INSTRUCTED TO PROVIDE COVERED WASTE RECEPTACLE.

Observed exposed porous wood at front counter and shelves behind counter. (counter has large openings where polyurethane has been poured but not sealed.) Must make all surfaces non-porous, smooth and easily cleanable. Must maintain same.

Must provide thermometers in all refrigeration units in a conspicuous spot. Must maintain same.
