OBSERVED AN ELECTRIC INSECT KILLER DESIGNED FOR OUTDOOR USE PLACED ABOVE THE PREP TABLE IN THE FOOD PREP AREA. MANAGEMENT INSTRUCTED TO REMOVE THE DEVICE FROM THE FOOD PREP AREA AND ONLY USE APPROVED INSECT KILLING DEVICES INSTALLED BY THE PEST COMPANY.
OBSERVED A FOOD HANDLING EMPLOYEE PREPARING FOOD WITHOUT A HAIR RESTRAINT. MANAGEMENT INSTRUCTED THAT ALL FOOD HANDLING EMPLOYEES MUST WEAR HAIR RESTRAINTS IN THE FOOD PREP AREA.

OBSERVED FROZEN MEAT STORED IN A PLASTIC GROCERY BAG IN THE FREEZER. MANAGEMENT INSTRUCTED TO USE ONLY FOOD GRADE STORAGE BAGS OR CONTAINERS.
CLEAN THE MICROWAVES, SPICE MIXERS AND DRY FOOD STORAGE CONTAINERS.
THE LEFT HAND SIDE BATHROOM DOOR LACKS A SELF-CLOSING DOOR DEVICE. MANAGEMENT INSTRUCTED TO INSTALL.

CLEAN THE WALLS OF THE FOOD PREP AREA AND FLOOR UNDERNEATH THE COOKING EQUIPMENT.
OBSERVED THE CERTIFIED FOOD MANAGER WITHOUT AN ALLERGEN TRAINING CERTIFICATE. MANAGEMENT INSTRUCTED THAT ALL CERTIFIED FOOD MANAGERS MUST SHOW PROOF OF ALLERGEN TRAINING.
