INSPECTION HISTORY
All food establishments that display, prepare, or store potentially hazardous food shall have adequate refrigerated food storage facilities. COOLERS AND FREEZERS OFF, MUST KEEP AT 40F OR BELOW.
In food establishments, there shall be adequate sewage and waste water disposal facilities that comply with all the requirements of the plumbing section of the Municipal Code of Chicago. MUST PROPERLY INSTALL A MOP SINK WITH HOT/COLD WATER AND BACK FLOW DEVICE,
Adequate and convenient toilet facilities shall be provided. They should be properly designed, maintained, and accessible to employees at all times. MUST PROPERLY INSTALL AN EXPOSED HAND SINK FOR BAR AREA WITH HOT/COLD WATER, SOAP AND HAND DRYING DEVICE.
Adequate and convenient hand washing facilities shall be provided for all employees. MUST PROVIDE SOAP AND SANITARY HAND DRYING DEVICE AT KITCHEN EXPOSED SINK AND WASHROOMS.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. MUST CONTRACT A LICENSED PEST CONTROL CO. AND PROVIDE LOG BOOK UPON REINSPECTION.
The area outside of the establishment used for the storage of garbage shall be clean at all times and shall not constitute a nuisance. MUST CLEAN OUTSIDE GARBAGE AREA AND PROVIDE CONTAINERS.
All utensils shall be thoroughly cleaned and sanitized after each usage. MUST CLEAN EQUIPMENT THRU-OUT PREMISES.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST CLEAN FLOORS THRU-OUT PREMISES.
The walls and ceilings shall be in good repair and easily cleaned. MUST SEAL OPENINGS AROUND PIPES IN KITCHEN.
Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. MUST PROVIDE SELF-CLOSING DEVICES FOR WASHROOMS DOORS.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST SEAL RUSTY GREASE TRAP UNDER KITCHEN'S 3PART SINK. ALSO MUST PROVIDE FILTERS FOR HOODS.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. MUST PROVIDE THERMOMETERS FOR COOLERS.
