6-501.111(E):OBSERVED THE PEST CONTROL LOG BOOK ON SITE MISSING INFORMATION. MUST PROVIDE. Every retail food establishment required to be licensed under chapter 4-8 of the Municipal Code of Chicago shall maintain a Pest Control Log Book on its premises containing the following pest control information: (1) A record of all visits to the retail food establishment by any pest control company describing all pest control-related activities conducted, including inspections and applications of pesticides or other methods to control pests; (a) All retail food establishments shall contract with a licensed exterminator who shall provide insect and vermin services at the establishment at least twice a year. (2) The log book record shall include a copy of the service ticket that sets forth the pest control company’s Illinois Department of Health license number, and the following: (a) Written verification (i.e. signature and certification number) of the certified technician responsible for using pesticides, or overseeing the use of pesticides by non-certified personnel; (b) Date of application; (c) Specify the targeted pest; and (d) Pesticide use recorded in the following manner: (i) Brand or common name (ii) USEPA Registration Number (iii) Percent active ingredient in the finished product (iv) An estimate of the amount of finished product used (3) A record of all locations of all applications of pesticides, trap and monitoring stations placed or monitored during the inspection; and (4) A record of any recommended corrective action needed to eliminate the condition, such as housekeeping deficiencies, pest harborages, or structural deficiencies. (5) All information in the log book shall be kept for a period of twelve (12) months. PRIORITY FOUNDATION VIOLATION#: 7-38-020(C). NO CITATION ISSUED.
