Source of contamination found inside ice machine. Found black and red substance inside interior panel of ice machine appear to be mold and algae. Condensation found around parts inside interior dipping black substance on fresh ice. Instructed to have interior of ice machine properly cleaned and sanitized by professional. "Held for Inspection" tag placed on machine. Critical citation issued 7-38-005(A).

Floor behind and under storage shelves and inside coolers not cleaned. Instructed to detail clean and remove kick plates to properly clean daily.

Ceiling tile and walls not maintained through-out premises. Instructed to replace all damaged ceiling tile, detail clean light and ventilation covers and repaint walls in rear missing and chipped paint. Tile coving along step in rear missing. Instructed to replace damaged and missing tile.
