Toilet facilities not in adequate number, convenient, accessible or maintained. Both employees and customers toilets not working. Establishmnet has three toilet facilities and one toilet will not flush and employees not able to use toilet facilities. Instructed to repair toilet facility and have all facilities working in good condition and must all toilets secured and properly bolted to floor and/or wall.
Deep freezer chest lid is not maintained. Instructed to replace or repair lid.
Can opener attached to prep table, exhaut hood and filters, sinks, coolers, utensils, cooking equipment and storage shelves not cleaned. Instructed to detail clean and sanitize daily.
Floor along wallbase and behind equipment not cleaned. Insstructed to detail clean daily and remove grease and food debris.
Walls not maintained in prep area and storage area. Instructed to detail clean walls and repaint. Rubber coving along wallbase in prep area missing. Instructed to replace rubber coving. Light covers in prep area not cleaned. Instructed to detail clean. Ceiling tile not maintained in rear storage area. Instructed to replace damaged ceiling tile.
Sink stoppers not provided for three compartment sink. Instructed to replace missing sink stopers.
Thermometer not provided inside display or prep cooler. Instructed to provide thermometer conspicuous inside all coolers.
