THEY ARE AS FOLLOWS...APPROXIMATELY 3LBS EGG SALAD, 10 OZ HUMMUS, 3 LBS OF DELI MEAT (HAM), 1 LB TUNA , AND 2 LBS OF COOKED FALAFEL ALL LOCATED INSIDE TWO DOOR REACH IN COOLER IN REAR PREP AREA. INSTRUCTED TO HOLD ALL TCS FOODS @ 41F OR BELOW. MUST MAINTAIN.ALL ITEMS TOTALED $41.95. MANAGER DISCARDED AND DENATURED ALL SAID ITEMS. PRIORITY VIOLATION. 7-38-005. CITATION ISSUED.
OBSERVED EXCESSIVE DRIED FOOD DEBRIS IN THE CRACKS AND CREVICES OF HOT HOLD UNIT LOCATED IN FRONT OF FACILITY. INSTRUCTED TO CLEAN AND MAKE FREE OF ACCUMULATED FOOD DEBRIS. MUST MAINTAIN.
OBSERVED PERSON IN CHARGE WITH ALLERGEN TRAINING CERTIFICATE ON SITE. NOTED ADDITIONAL 3 OTHER CITY OF CHICAGO CERTIFIED FOOD MANAGER CERTIFICATE HOLDERS ON SITE WITHOUT ALLERGEN TRAINING COMPLETED. INSTRUCTED MEET REQUIREMENT FOR ALL MANAGERS AND OBTAIN ON SITE FOR REVIEW.
