OUTER OPENING NOT PROTECTED.FOUND FRONT BOTH DOORS WIDE OPENED.INSTRUCTED TO KEEP DOORS CLOSED AT ALL TIMES TO MINIMIZE PEST/INSECTS ENTRY. OBSERVED EVIDENCE OF RODENT DROPPINGS(100 OR MORE DROPPINGS) IN DIFFERENT LOCATION OF PREMISES,FIRST FLOOR PREP AREA; INSIDE THE BOTTOM OF DEEP FRYER,INSIDE THE OVEN, ON BOTTOM PANELS OF DISPLAY COOLER BY THE THREE COMPARTMENT SINKS,ON TOP THE SHELVES BY THE OPENED FOOD CONTAINERS, AROUND THE RICE STEAM POT,AND BETWEEN SPICE CONTAINERS. DROPPINGS WAS OBSERVED UNDER THREE COMPARTMENT SINKS, ALONG WALLS,CORNERS AND UNDER SHELVES,ON TOP OF CARDBOARD THAT COVERS THE GREASE TRAP BY THE COOKING EQUIPMENT. FRONT COUNTER,UNDER PHONE WIRES AND PLASTIC BAGS. OBSERVED MICE DROPPINGS ON SECOND FLOOR(STORAGE AND WALK-IN COOLER AND WALK-IN FREEZERS IS INSTALLED AT SAID AREA) UNDER THE SHELVES. ALSO UNABLE TO CHECK ALL AREA DUE TO A LOT OF BOXES AREA STORED ON FLOOR . INSTRUCTED TO REMOVE DROPPINGS,CLEAN AND SANITIZE AREA MENTIONED. ELEVATED ITEMS STORED ON FLOOR, 6" OFF THE FLOOR, INCLUDED THE OFFICE AREA. SERIOUS VIOLATION: 7-38-020.
