Observed hand sinks without paper towels in the following locations: 2 hand sinks in kitchen, 1 hand sink in server station on 2nd floor. Observed utensils stored inside sink basin in the kitchen near the cook line. Instructed hand sink may only be used for hand washing. Instructed must provide hot & cold water, adequate city pressure, soap and paper towels at all hand sinks at all times. Items removed from hand sink basin, and all hand sinks stocked with paper towels during inspection. Critical violation 7-38-030.
VACUUM PACKAGING EQUIPMENT ON PREMISES WITHOUT REGULATORY APPROVAL. MACHINE HAS BEEN TAGGED 'HELD FOR INSPECTION' BY THE CHICAGO DEPT OF PUBLIC HEALTH. INSTRUCTED NOT TO USE UNTIL APPROVAL FROM CDPH IS PROVIDED. VACUUM PACKAGED MEATS VOLUNTARILY DISCARDED DURING INSPECTION. SERIOUS 7-38-005(A).
Observed items without labels in prep coolers. Must properly label all potentially hazardous foods with date when held for more than 24 hours. Must maintain same. No consumer advisory on menu or posted. Instructed must post in plain view or add to menu.
Observed glasses upright and silverware unprotected on tables in dining room. Must invert all glasses and cover silverware (or partially cover) to prevent contamination. Must maintain same.
Observed table top can opener with excessive food debris. Must detail clean and sanitize. Must maintain same.
Observed low water pressure (cold water) at bar hand sink and at 1 compartment sink in server station on 2nd floor. Must provide adequate water pressure at all plumbing.
Observed items stored directly on the floor in storage rooms in basement. Must elevate all items at least 6 inches above the floor to allow proper cleaning. Observed mops stored inside utility sink (downstairs). Must hand all mops to dry. Must maintain all.
Observed several food handlers without proper hair restraints while in the food prep area. Must provide hair restraints (hats, hair nets, etc.) for all food handling employees. Must maintain same.
Must provide proof of food handler training for all food handling employees.
