Instructed to remove all pest activity, detail clean and sanitize areas, rodent proof front door to prevent rodent and insect entry and have licensed pest control service premises and provided current service invoice. Rear door propped-open to outside without screen. Must keep doors closed or install tight-fitting screens. Seal openings to outside at side door.
Dish washing facilities not maintained or operated. While running and draining water in middle of three compartment sink, water coming up left and right sides of sink-food particles and black gritty substance. Instructed to detail clean and/or repair grease trap to prevent inadequate disposal of waste water.
