OBSERVED A GAP UNDERNEATH THE FRONT DOOR AND IS NOT RODENT/INSECT PROOFED. INSTRUCTED TO RODENT/INSECT PROOF THE FRONT DOOR AND MAINTAIN.
OBSERVED THE FOOD HANDLER NOT WEARING A HAIR RESTRAINT. INSTRUCTED TO OBTAIN AND MAINTAIN WEARING A HAIR RESTRAINT WHEN PREPARING/SERVING FOODS OR WHILE IN THE FOOD PREP AREA.
OBSERVED A WIPING CLOTH NOT PROPERLY STORED, WIPING CLOTH FOUND ON THE STORAGE RACK. INSTRUCTED TO STORE WIPING CLOTHS IN SANITIZING SOLUTION (MUST BE AT THE REQUIRED PPM OF THE SANITIZER USED) WHEN NOT IN USE. MAINTAIN SAME.
OBSERVED THE CUTTING BOARD ON THE PREP COOLER IN FRONT OF THE COOKING EQUIPMENT WITH STAINS AND DEEP, DARK GROOVES. INSTRUCTED TO SAND OR REPLACE CUTTING BOARDS TO MAKE THEM SMOOTH AND EASILY CLEANABLE. MAINTAIN SAME.
OBSERVED THE LOWER SURFACE IN THE INTERIOR AND THE RUBBER GASKETS OF THE PREP COOLER WITH FOOD AND DIRT. INSTRUCTED TO CLEAN AND MAINTAIN COOLER.
OBSERVED THE CEILING TILE OUTSIDE OF THE TOILET ROOM WITH A WATER STAIN. INSTRUCTED TO REPLACE WATER STAINED CEILING TILE AND MAINTAIN.
OBSERVED THE FLOORS IN THE CORNERS, UNDER/AROUND THE COOKING/COOLING EQUIPMENT, GREASE TRAP AND SINKS WITH A BUILD UP OF FOOD, DIRT AND DEBRIS. INSTRUCTED TO DETAIL CLEAN AND MAINTAIN FLOORS THROUGHOUT. MUST CLEAN AND MAINTAIN THE WALLS IN THE FOOD PREP AREAS TO REMOVE DIRT AND FOOD SPLATTERS.
OBSERVED THE VENTILATION HOOD WITH GREASE AND DIRT. INSTRUCTED TO CLEAN AND MAINTAIN VENTILATION HOOD.
