OBSERVED MULTIPLE SOURCES OF WATER LEAKS FROM THE CEILING, HVAC SYSTEM AND STEAM LINES ABOVE THE REAR STORAGE ROOM WHERE FOOD IS STORED. ALSO OBSERVED PEELING AND CHIPPED PAINT ON THE WALL BELOW THE HVAC DUCTS. MANAGEMENT INSTRUCTED TO PROVIDE A CEILING ABOVE THIS AREA AND REPAIR SOURCE OF ALL WATER LEAKS. MUST STORE ALL COOLERS AND FREEZERS UNDER THE PROTECTED CEILING AREA AND REMOVE THE STANDUP COOLER FROM UNDERNEATH THE WASTE WATER LINES. ALSO REMOVE ALL PACKAGES AND CONTAINERS FROM THE STORAGE RACKS UNDERNEATH THE WASTE WATER LINES. SERIOUS VIOLATION 7-38-005A.
When toilet and lavatory facilities are provided for the patrons of food establishments, such facilities shall be adequate in number, convenient, accessible, properly designed, and installed according to the municipal code. WOMEN'S RESTROOM IS IMPROPERLY MAINTAINED. OBSERVED A CRACK AT THE BOTTOM OF THE TOILET THAT IS LEAKING WATER ONTO THE FLOOR. MANAGEMENT INSTRUCTED TO PUT AN OUT OF ORDER SIGN ON THE WOMEN'S RESTROOM. MUST REPLACE THE TOILET. SERIOUS VIOLATION 7-38-030.
REPLACE THE DOOR TO THE REAR STORAGE AREA AND PROVIDE A DOOR SWEEP AT THE BOTTOM OF THE DOOR. MUST INSTALL A SPLASHGUARD IN BETWEEN THE EXPOSED HANDSINK AND THE 3 COMPARTMENT SINK IN THE REAR PREP AREA.

OBSERVED STANDING WATER IN THE WOMEN'S RESTROOM AND UNDERNEATH THE DISHMACHINE. MUST MAINTAIN THE FLOOR DRY THROUGHOUT. MUST REPAINT THE FLOOR IN THE REAR STORAGE AREA.
The walls and ceilings shall be in good repair and easily cleaned. REPLACE THE MISSING WALL TILES IN THE WOMEN'S RESTROOM AND WALL IN REAR STORAGE AROUND THE ICE MACHINE. MUST SEAL STAINLESS STEEL WALL SECTION TO DRYWALL AROUND THE MOP SINK. REPLACE THE WATER STAINED CEILING TILES IN THE HALLWAY TO THE RESTROOMS AND THE MEN'S RESTROOM.
OBSERVED OPEN DRAIN LINE NEXT TO THE 1 COMPARTMENT SINK IN THE REAR STORAGE ROOM. MUST CAP AND SEAL. 1 COMPARTMENT SINK IS ALSO SLOW DRAINING. MUST REPAIR OR REMOVE SINK. DRAIN LINE FOR THE DISHMACHINE IS IMPROPERLY MAINTAINED SO THAT THE WATER DOES NOT DRAIN INTO THE FLOOR DRAIN. MUST REPAIR.
