All dishwashing machines must be of a type that complies with all requirements of the plumbing section of the Municipal Code of Chicago and Rules and Regulation of the Board of Health. OBSERVED 3- COMP SINK 2ND FAUCET WITH SPRAYER NOT ABLE TO STAY ON TO FILL SINK FOR USE. MUST CONNECT FAUCET HEAD SO THAT WATER CAN FILL 2ND AND 3RD SINK.

All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. MUST CLEAN/SANITIZE ALL FOOD CONTACT SURFACES.

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. OBSERVED FLOORS NOT CLEAN IN BASEMENT,MUST CLEAN/SANITIZE FLOORS FROM DEBRIS. ALSO MUST SEAL OPENINGS IN FLOORS,ALONG WALLS IN REAR STORAGE GARAGE AREA AND REAR SHIPPING AREA.

The walls and ceilings shall be in good repair and easily cleaned. OBSERVED CEILING AREAS IN REAR SHIPPING ROOM IN POOR REPAIR /LEAKING WATER.MUST REPAIR. ALSO,OBSERVED OPENING/CRACK-CREVIES THRU-OUT PREMISES.(BAKERY PREP,SHIPPING ROOM,INSIDE GARAGE PORCH AREA).

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. OBSERVED BOTTOM OF 3-COMP SINK AROUND RIM OF PIPE LEAKING,AND HOT WATER HANDLE OF FAUCET IN WASH ROOM,NOT ABLE TO OPEN FOR USE OF WATER,MUST REPAIR.ALSO, OBSERVED NO FAN INSIDE VENTILIATION STACK TO PUSH AIR OUT,MUST INSTALL FOR USE OF BBQ PIT.

All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. OBSERVED STORAGE ON FLLOORS,AGANIST WALLS IN BASEMENT STORAGE,REAR INSIDE GARAGE STORAGE AREA,CLUTTER. MUST PROPERLY STORE ARTICLES ETC, OFF FLOORS AND WALLS AREAS 6-INCHES OFF FLOORS TO PREVENT A PEST HARBORAGE AREA.
