NOTED ANOTHER EMPLOYEE ON THE PHONE, WIPE HANDS ON HIS PANTS AND CONTINUES TO TEH KITCHEN TO PREPARE FOOD WITHOUT WASHING HANDS OR WEARING GLOVES. NOTED ANOTHER EMPLOYEE WHILE WEARING GLOVES AND CHANGING THEM DID NOT WASH HANDS BETWEEN TASKS. INSTRUCTED MANAGEMENT THAT ALL EMPLOYEES MUST WEAR GLOVES, WASH HANDS AND CHANGE GLOVES BETWEEN TASKS. OBSERVED LAUNDERED LINENS BEING STORED IN A DIRTY OVEN. INSTRUCTED TO STORE LINES IN A CLEAN, DRY LOCATION.

OBSERVED EMPLOYEES WITH CITY OF CHICAGO SANITATION CERTIFICATES WITHOUT ALLERGEN CERTIFICATES. INSTRUCTED ALL EMPLOYEES WITH CITY OF CHICAGO SANITATION CERTIFICATES MUST COMPLETE ALLERGEN TRAINING AND MAINTAIN COPIES OF THEIR CERTIFICATES ON PREMISES AT ALL TIMES.
