FOOD (ICE) NOT PROTECTED DURING STORAGE INSIDE OF THE ICE MACHINE. OBSERVED BLACK SLIMY SUBSTANCE ON INTERIOR WATER COMPONENTS OF THE ICE MACHINE. MANAGEMENT INSTRUCTED TO TURN OFF MACHINE, REMOVE ICE AND HAVE INTERIOR OF ICE MACHINE CLEANED AND SANITIZED.
The basement rear exit door has 1 inch gap across bottom. Instructed to rodent proof door to prevent rodent and insect entry.All rodent bait must be properly stored in a sanitary manner inside enclosed bait stations.Also, establishment does have pest control logbook on premises. Instructed to remove exposed rodent bait pellets and have licensed pest control service the premises and provide logbook with all required documents include a map locator to identify the pest control services provided for this food establishment.
Splashguards are needed for the exposed handsinks and sink stoppers are needed for the 3 compartment sink.
Seal openings on walls and around plumbing/electrical pipes throughout the basement storage areas.
A backflow prevention device is needed for the ice machine.
Thermometers are needed for all refrigeration units.
Unnecessary articles must be removed from the premises due to potential pest harborage.
